In order to attend any event with the Ave Maria Youth Group in 2010-2011, you must first register. Note: Registration entails the purchase of a $12 youth group t-shirt which you will wear to youth group events. Also, a $50 credit will be awarded to the best t-shirt design, so put your design on paper and drop it off at the Donahue Academy office.
To register, first print out and fill out a Release Form and a Medical Form. These forms must be notorized, which may be done at Qwik Pack and Ship in Ave Maria. Please give these forms to Ed Rivera (239) 287-3215. Second, fill out the form below with your personal information for our records and your safety.
